TO: Zen Math (the “Business”)

FOR GOOD AND VALUABLE CONSIDERATION, THE RECEIPT AND SUFFICIENCY OF WHICH I HEREBY ACKNOWLEDGE, I, THE UNDERSIGNED, HEREBY AGREES TO THE FOLLOWING:

1.   I understand that my oral statements, voice, image and likeness, and any material based upon or derived from any testimonials given to the Business (collectively, the “Testimonials”) will be recorded by various written or digital means of all descriptions by the Business or those contracted by the Business. 

2.   I hereby consent to the use of the Materials in connection with the production of advertising and promotional materials (collectively, the “Business Materials”) that may be developed to promote the Business and any related businesses.  I hereby grant to the Business the full and complete right in perpetuity, to use the Testimonials, either alone or accompanied by other material, in the Business Materials for the purpose of advertising, promoting and publicizing the Business.

3.   I understand that I will have no right of approval, no claim to additional compensation or benefit, and shall not hold the Business responsible for any liability resulting from the use of the Testimonials in accordance with the terms hereof, including, but not limited to any claims based upon breach of a non-disclosure agreement, invasion of privacy publicity, false endorsement or defamation.

4.   I acknowledge that the Business may use and store my personal information for administration purposes in accordance with their Privacy Policy.

5.   This agreement is governed by the laws of the Province of Ontario and the federal laws of Canada applicable therein.
